The Big Plague is one of Bob's minions, shown below Swarm of Bats and above The Black Lich. He has green skin like Bob, a chubby body type, light armor, and some cloth. He shoots green balls of concentrated plague. You don't know this new me; I put back my pieces differently.
It seems to be a reference to the Great Plague that was the most devastating pandemic in the history of mankind during medieval times.
To have access to The Big Plague one needs to have Swarm of Bats at least at level one.
