Nightmare Book is a Nightmare item that can be found in Nightmare Mode Beta. It was added to the game in v1.03.053, but is currently unobtainable.

This item used to add 5% to The Black Lich DPS for each Black Orb you have. The effect stacks multiplicatively, unlike the Lich's ABC's. It currently has no effect.
